SUPR has released the Substance Abuse Block Grant Survey for SFY22. This survey is mandatory for all funded providers and must be completed by October 27, 2021.

The Illinois Department of Human Services/ Division of Substance Use Prevention and Recovery (IDHS/SUPR) requires that each funded agency complete the SFY2022 Substance Abuse Prevention and Treatment Block Grant (SABG) Survey. SUPR uses survey information in combination with other data to measure priority areas identified as critical to our system. SUPR utilizes this information to report back to the Substance Abuse and Mental Health Services Administration (SAMHSA) on priority areas and our progress on addressing unmet service needs and gaps in our system.

Your participation is required to meet contractual obligations. The survey should take no more than ten minutes to complete and should only be submitted once by each funded agency. To access the survey, please visit Substance Abuse Prevention and Treatment Block Grant (SABG) Survey.
